A client sends a reference image and asks how much. No placement. No size. No idea a deposit comes first. You can't quote that yet. An intake form asks the questions for you, every time.

can you do this?? how much Your intake form is custom. You pick the questions, the order and the field for each answer. Put the question that moves the quote first.
A short answer for the idea. A dropdown when there are only a few choices. An upload for the reference image. A Placement question, where you pick how clients answer: just ask, a 2D map or a 3D body. A checkbox for your deposit terms.
Keep it short enough to finish on a phone. Every extra field is a reason to close the tab and DM you instead.
Without a form, the details come in pieces: a picture on Monday, "arm i think" on Wednesday, a deposit question on Friday. You scroll back through the thread to write the quote.
With an intake form, it comes in as one request with the reference image, placement and size filled in.

Hi Jess, a palm-sized floral on your right outer forearm works. Your quote is on the way. Once I approve the booking you'll get a link for the $20 deposit.
The intake form is the first piece of paper. Paperwork holds the rest: documents, waivers, licences, BBP certificates and taxes in one area.
Clients sign a digital waiver, and their photo ID sits on the client record next to the intake form. In a studio, each artist's licence and BBP certificate sits in the same place, so you can see whose is on file.

If an answer changes the quote, it belongs on the form. If it doesn't, cut it. Short forms get finished.

Back to Jess and her floral printout. The reference image shows the style. It doesn't say where it goes or how big.
What Jess pickedOn your intake form, the client uploads the image, then marks placement on a 2D or 3D body and picks a size. You open the request and see the design next to where it goes.
