Tattoo intake forms that get you the details you need

A client sends a reference image and asks how much. No placement. No size. No idea a deposit comes first. You can't quote that yet. An intake form asks the questions for you, every time.

What to ask before you quote anything

Every quote needs the same five answers. Without them you're guessing, and the client holds you to the guess. Put them on your intake form once and stop typing them into DMs.

  1. The reference imageWhat they want. A picture shows the style faster than a paragraph.
  2. PlacementWhere on the body, which side, inner or outer.
  3. SizeA coin, a palm, or bigger. A palm piece and a sleeve are different sessions.
  4. Deposit and sessionsSay on the form that a deposit comes after you approve, so nobody is surprised.
  5. Anything that changes the stencilA cover-up, work nearby, a name to spell.

If an answer changes the quote, it belongs on the form. If it doesn't, cut it. Short forms get finished.

Your CREW.INK intake form is custom. You pick the questions, the order and the field for each answer. Put the question that moves the quote first.

A short answer for the idea. A dropdown when there are only a few choices. An upload for the reference image. A Placement question, where you pick how clients answer: just ask, a 2D map or a 3D body. A checkbox for your deposit terms.

Reference images and placement

Back to Jess and her floral printout. The reference image shows the style. It doesn't say where it goes or how big.

On your intake form, the client uploads the image, then marks placement on a 2D or 3D body and picks a size. You open the request and see the design next to where it goes.

Without a form, the details come in pieces: a picture on Monday, "arm i think" on Wednesday, a deposit question on Friday. You scroll back through the thread to write the quote.

With an intake form, it comes in as one request with the reference image, placement and size filled in.

Hi Jess, a palm-sized floral on your right outer forearm works. Your quote is on the way. Once I approve the booking you'll get a link for the $20 deposit.

What the client experiences

For the client, it's one link, a short form, then a deposit once you say yes.

They know the price before they pay. You know the design, placement and size before they walk in.

  1. They open your intake form, on your own domain and brand colors if you set them up.

  2. They upload a reference image, mark placement and answer your dropdowns.

  3. You read the request and send a quote. No deposit is taken until you approve the booking.

  4. They get a text with the deposit link and pay $20 by card. The client pays the $2.50 booking fee by default, or you split it 50/50 or cover it.

  5. Reminders go out by SMS and email 2 days, 2 hours and 30 minutes before the appointment. You can add 7 days and 1 day.

Waivers, licences and taxes in Paperwork

The intake form is the first piece of paper. Paperwork holds the rest: documents, waivers, licences, BBP certificates and taxes in one area.

Clients sign a digital waiver, and their photo ID sits on the client record next to the intake form. In a studio, each artist's licence and BBP certificate sits in the same place, so you can see whose is on file.
